Home care services focus on recovery, quality of life, and independence with the goal to reduce ER visits and hospital readmissions.
For more than 100 years, our skilled nursing team has been providing much needed nursing care to Mainers in their homes. Our highly skilled nurses, rehabilitation therapists (speech, occupational, physical), social workers and home health aides offer expertise in managing complex patient needs and work as a team to provide a coordinated continuum of care.
When called in by your physician, our nurses will review your medications, monitor vital signs, treat wounds, monitor overall health and provide other health related care. Their findings, as well as any concerns they may have about your health, are communicated directly to your physician.
Northern Light Health also provides the support of a licensed clinical social worker to assess your in-home needs and abilities to make certain that your plan of care includes all the support services that you really need.
We offer a wide array of solutions to your therapy needs, whether they be physical, occupational or speech and language-related. Our experienced team of therapists, who are specifically trained in home health care, understand the value of a collaborative approach.
That’s why we all work together, no matter our specific area of focus, to find the best course of treatment for every patient.
