Northern Light Transitional Care offers a full range of services, tailored to the needs of individual patients and their loved ones.
Our services are designed to promote rapid and lasting recovery after a stroke, surgery, major illness, or accident. By coordinating care, our onsite services will enable you to return home and to work as soon as possible.
Patients are typically admitted to our Transitional Care program directly upon discharge from a hospital stay or within 30 days of discharge. We also work with patients from nursing homes and other organizations.
Once you have been admitted to our program, our interdisciplinary team works with you to develop an individualized plan of care. We also work with loved ones to ensure that the transition to home, or another care environment, is as smooth as possible.
Transitional Care is often covered by Medicare and Medicaid, as well as many private insurance plans. We also have a financial assistance program for those who qualify.
Home Safety Evaluations
Our therapists perform home evaluations on a case-by-case basis to determine if a patient can safely return home. They assess a patient’s ability to move and function in familiar surroundings, including their bedroom, main living area, bathroom, kitchen, and outdoor environment.
This process allows therapists to make recommendations to increase safety, decrease the risk of falls, and provide patients with techniques and adaptations to increase their independence.
