Our Health Information Management team helps patients, providers, and insurance companies access records as allowed by law.

Request Medical Records

Your medical records include details about your care, including visits, test results, and treatments. This information helps you understand your care and supports safe coordination across the healthcare system. You may request medical records from Northern Light Health hospitals or facilities.

How to Request Medical Records

Many records are part of your electronic health record and may be available through the My Northern Light Health patient portal, which gives you secure access to your health information online.
 
You may request medical records by completing the Authorization to Release Healthcare Information form and submitting it by email, fax, or mail. Be sure to select the facility or facilities you are requesting records from. You may choose more than one location.

Request a Change to Your Medical Record

Ensuring your patient health information is accurate and complete helps ensure you receive the highest quality care. If you believe something in your medical record is incorrect or missing, you may write to request a change. Your request will be reviewed by the provider who created the record, and Health Information Management professionals will send you a written response within 60 days. If a request is denied, you may submit a written statement of disagreement. All related documents become part of your medical record.
 
To request a change, complete the Medical Record Amendment Request form and submit it to Health Information Management using the contact information below.

Request Records for a Deceased Patient

Authorized individuals may request medical records for a deceased patient. To release these records, you will need to submit required documents, including an Authorization to Release Healthcare Information form and proof of authority, such as a death certificate, or probate court paperwork. These steps help protect patient data and privacy.

Contact Health Information Management

Our Health Information Management team is available to answer your questions and assist with records requests. Notary services may also be available. If you call after 5pm, please leave a message and a team member will return your call on the next business day.

Many patients can access parts of their electronic health record through the patient portal. Visit MyNorthernLightHealth.org to learn more.

If you have questions regarding a request for medical records or are requesting records for someone other than yourself, please call the Health Information Management department at the respective hospital or other member organization listed below. Completed authorization forms, advance directives, and power of attorney forms may be submitted to the appropriate department or member organization via mail, fax, or email.

If you cannot speak for yourself, an advance directive and power of attorney forms allow you to share your healthcare wishes. These documents help support clear communication with your healthcare teams and guide care decisions.

Health Information Sharing and Technology

Northern Light Health participates in secure information technology networks that support care coordination.

HealthInfoNet is Maine’s health information exchange, which helps connect patient care records at facilities across the state. It allows approved providers to view key patient data, such as medications and allergies, when caring for you. Participation is voluntary, and you may opt out.

You can choose not to participate in HealthInfoNet. Participation is voluntary. If you feel the risks outweigh the benefits, you may choose not to participate ("opt-out"). For more information about HealthInfoNet, please visit their website below or contact 866-592-4352.

CommonWell

CommonWell is a nationwide information management association that helps providers access medical records across care locations throughout the US allowing practitioners to make more informed healthcare decisions and coordinate your care across care teams nationwide. Information is encrypted, protected, and never sold. 

You can choose not to participate in the CommonWell network. Participation is voluntary. If you feel the risks outweigh the benefits, you may choose not to participate ("opt-out").  To opt out of CommonWell, call the Patient Help Desk (833) 217-9640.
