Vaccine Smart Card

Our SMART Health Card feature, available through our patient portal website and app, allows you to share a limited amount of information, such as a vaccine record, with verifiers portably and securely.

To access your SMART Health Card, log into MyNorthernLightHealth, our patient portal, from your phone, tablet, or desktop computer and select COVID Center from the left-hand menu.

Next, select the View Vaccine Verification button at the top, center of the page.
 


This takes you to your vaccine verification page where you can print your SMART Health Card, screen cap the card for offline use, or scan the QR code to upload your vaccination to a compatible vaccination passport program or your app of choice.

More Information:
  • SMART Health Cards generated in this system meet SMART Health Card standards as part of a US and Global effort to define a digital vaccination card standard. To learn more about this coalition, you can visit VCI.org.
  • If you have patient portal access and received your COVID-19 vaccination at Northern Light Health, you have access to the SMART Health Card feature. Neither you nor your healthcare provider need to take any steps to add this feature and there is no cost to use this feature.
    • At this time, if you received your COVID-19 vaccination from a non-Northern Light Health provider or pharmacy you would need to contact them for access to their electronic verification service.
  • To view a vaccination SMART Health Card for someone else, such as a child or family member, you must be authorized as a proxy user in that person’s patient portal. To learn more about proxy access, whether or not you can get proxy access, and to complete forms for proxy access, please speak with your healthcare team at your next appointment.

  • SMART Health Cards cand be loaded to many third-party applications for easy access (e.g. Apple Wallet, GooglePay, SamsungPay, etc). To add your card to one of these apps you will need to follow the instructions provided by your app/operating system of choice. We are unable to provide technical support for these apps and mobile device operating systems.
