We require that your primary care or other physician make a referral for your first appointment, and will ask for them to send us any pertinent records from your medical chart. For your first evaluation, patients meet with a behavioral medicine psychologist to better understand the situation and a plan can be developed to include one or more of the following:
A series of individual appointments, usually 6-12 visits
One time consultations and planning
Flexible treatment planning for those who live far away
Referral to additional supports and resources
We strive to keep appointments running on time. We realize that you may experience an emergency that causes you to be late or miss an appointment. Please be aware that you may be asked to reschedule if you arrive more than 15 minutes late for your scheduled appointment. If something comes up, please contact the office as soon as possible to allow us to fill that appointment time with another patient. A 24-hour cancellation notice is appreciated.
Pre-registering for appointments allows us to have your paperwork ready when you arrive. Please call EMMC's Pre-Registration Hotline toll-free at 1-888-973-8900 or if your call is local, 973-8900. Representatives are available Monday through Friday 7 am to 7 pm. You can leave a message after hours and you will receive a call back from a representative. Our practice is dedicated to your privacy and confidentiality. By law, we are only able to speak to the patient or medical power of attorney/legal guardian unless an authorization to release health care information (ROI) form has been completed.
