Leigh Ann Howard RN, MSN, CHFN and Director of Home Health and Specialty Programs at Northern Light Home Care & Hospice.
Ms. Howard has more than 17 years’ experience in the health care industry with most of those years in home based care. In her current role she directs the home health, telehealth and community wellness programs. She works closely with her senior management team to develop new and innovative telemedicine and home health programs to achieve the Triple Aim. She has gone “outside the home care box” to work with Medicare Advantage plans and ACO groups to help reduce costs and improve patient care.
Her current role also involves community wellness. This involves a large influenza vaccination program at most schools in Maine’s two most southern counties. This work lead to the article Best Practice Model for Immunization: One Home Care Agency's Experience Providing H1N1 and Seasonal Vaccine
published in Home HealthCare Nurse: Journal for the home care and hospice professional. She received a Leadership Award from the American Nurses Association in 2010 for her immunization work. She is also published in Clinical Case Studies in Home Health Care.
Ms. Howard received an MSN and is certified in heart failure through the American Association of Heart Failure Nurses.
