Important Health Coverage Tax Documents

In compliance with the Affordable Care Act (ACA), Form 1095-C for the 2024 tax year is available to all full-time employees upon request. 

You are not required to file Form 1095-C with your tax return. However, if you would like a copy for your records, you may request one using the contact information below.
