||Job Summary:The Clinical Associate is an integral member of theinterdisciplinary team and works under the direction of the Senior PsychiatricClinician.The Clinical Associate is responsible for the delivery ofcare coordination to patients, individually or in-groups across the life spanconnecting patients to valuable outpatient resources in hopes of reducingrecidivism.The Clinical Associate functions for patients as thedischarge planner, supportive counselor, and educator and performs casemanagement activities.Additionally the Associate uses criticalthinking skills to assess and advocate for patients; families in order todevelop an individualized treatment and aftercare plan.The planidentifies desired outcomes and the strategies and resources to be used inattaining the outcomes.Qualifications:
Physical Demands/Environment:Visual acuity, color vision, hearing acuity, and tactile sensationare necessary to respond to patient response and requests. Has the ability toread and speak English, to communicate and document information. Possess themanual dexterity and a degree of body strength and flexibility to respond toaggressive patient behavior. Has the ability to reach and stretchintermittently. Time may be spent sitting at a computer, collaborating withother healthcare team members. - Bachelor's Degree Human Services or related field, LSW or LSW eligible, MHRT/C strongly preferred.
- Experience in hospital-based clinical treatment services preferred.
- Age based competency in children, adolescents, adults and geriatrics and disability based competency for chemical dependency required.
- Critical thinking, excellent time management and organization, cognitive flexibility, prioritize, multitask
- Computer skills (word, excel, database, outlook).
- Must possess good oral and written communication skills in the English language
