||POSITION SUMMARY: The Nurse Navigator serves as a consistent care coordinator for the diagnostic and surgical end of Gastroenterology care to ensure timely access and seamless coordination of services throughout the continuum. The Nurse navigator will interface with the schedulers, internal and external providers, and assist in the diagnostic imaging, clinical evaluation, support services, and education of patients receiving services. The nurse navigator will be responsible for facilitating and coordinating the care of patients of Gastro that receive their care from providers working in collaboration with Gastro. He or She will identify and help eliminate barriers and refer to appropriate resources. Additionally, the nurse navigator will be responsible for the triage and coordination of referrals sent to our office. They will coordinate local support systems, arrange patient focus groups, and collaborate & provide educational resources for patients, families, and staff. They will be responsible for assisting with clinical needs and acting as a resource for clinical staff.MINIMUM EDUCATION REQUIRED: Registered nurse with valid Maine license. Bachelor's degree preferredMINIMUM EXPERIENCE REQUIRED: Minimum two years nursing experience. Demonstrates strong clinical skills. Demonstrates effective interpersonal communication skills, teamwork, and organizational skills. Effective collaborative, problem solving skills. Develop and manage nursing staff competency requirements; and assist in the development and maintenance of departmental directives that relate to nursing process and procedure. Demonstrated ability to teach, write directives, and oversee written procedure adherence.PREFERRED EXPERIENCE: Preferred knowledge & experience with gastroenterology procedures.OTHER SKILLS REQUIRED: Computer literate and a strong team player. Must have a strong attention to detail. Must be able to communicate openly with patients, families, and providers. Must be able to multi-task, have strong interpersonal and problem-solving skills.PHYSICAL REQUIREMENTS: Ability to tolerate extended period of computer and telephone use. Must be able to assist in patient transfers from wheelchair to exam table, lift up to 50 pounds unassisted, continuous standing and walking, periodic bending and stooping.WORKING ENVIRONMENT: Clean, well-lighted, well ventilated, working in a busy office and close environment with others. Given the nature of the position, work is at times stressful.
