||Job Summary: The IntegratedBehavioral Health Home Coordinator (IBHC)works as part of an integrated multi-disciplinary team with adults whohave serious and persistent mental illness and children with serious emotionaldisturbance. The IBHC is integrated into departments and settings outside ofAcadia Healthcare and will serve as a representative of Northern Light AcadiaHospital and a consultative partner for the integrated practice. The IBHC provides comprehensive casemanagement services with a set of priorities aimed at; improving mental health,improving medical health (including ascertaining and effectively working with aprimary care provider and engaging in preventative and health promotionservices); reducing inpatient hospitalizations and emergency department visits;ensuring effective transitions during episodes of more intensive treatment;tracking client progress towards goals; and working within the team structureto continually refine and re-prioritize activities. Qualifications:
Physical Demands/Environment:Visual acuity, colorvision, hearing acuity, and tactile sensation are necessary to respond topatient response and requests. Has the ability to read and speak English, tocommunicate and document information.Possess the manual dexterity and a degree of body strength andflexibility to respond to aggressive patient behavior. Has the ability to reach and stretchintermittently. Time may be spent sitting at a computer, collaborating withother healthcare team members. Standing and walking comprises some of theworkday. Possess the ability to sit forlong periods of time in meetings and in vehicles. The work environment consistsof a range of settings including, but not limited to typical office settings,medical settings, schools, community agencies and any other partner locations.The setting may include computer, confidential medical records, medicaldocuments and communication channels, and may be somewhat stressful. There is also a community component with someof the casework occurring in the patient homes and other communitylocations. - Bachelor's Degree Human Services or related field required,
- MHRT/C or certification eligible within a reasonable period of time.
- Must possess good oral and written communication skills in the English language
- Job related experience in mental healthcare is (preferred)
- Ability to work with both clinical and non-clinical personnel in potentially high stress situations is required.
