||Job Summary:The BehavioralHealth Coordinator (BHC) works as part of a multi-disciplinary team with adultswho have serious and persistent mental illness and children with seriousemotional disturbance. The BHC provides comprehensive case management serviceswith a set of priorities aimed at; improving mental health, improving medicalhealth (including ascertaining and effectively working with a primary careprovider and engaging in preventative and health promotion services); reducinginpatient hospitalizations and emergency department visits; ensuring effectivetransitions during episodes of more intensive treatment; tracking clientprogress towards goals; and working within the team structure to continuallyrefine and re-prioritize activities.The BHC will have strong de-escalation skills, and be able to workquickly, flexibly, and efficiently. TheBHC will perform and assist with the performance of full psychosocial assessments, identificationof needs and resources, and developmentof individual treatment plans and crisis plans. The BHC is responsible formaintaining clinical documentation consistent with all licensing and regulatoryrequirements. The BHC will have strongorganizational skills, communication skills, and networking capabilities. The BHC is responsible for coordinating careand maintaining the records on a caseload of assigned clients. There is also a community component with someof the casework occurring in the patient homes and other communitylocations. The BHC transports clientsfor clinically appropriate reasons. Perform any duties/or responsibilitiesassigned by Administrator or Supervisor for which qualified.Job Requirements:
- Bachelor's Degree Human Services or related field required, MHRT/C (Mental Health Rehabilitation Technician) or certification eligible within a reasonable period of time.
Abilityto travel in assigned service area.
- Valid driver's license with reliable transportation and required auto liability insurance.
- Job related experience in mental healthcare is preferred with knowledge of psychiatric and chemical dependency terminology.
- Cognitive requirements including the ability to analyze a situation and make a sound decision on the best way to arrive at a solution.
