Job Details GENERAL DESCRIPTION/STATEMENT: The dialysis social worker works with dialysispatients and their families, in collaboration with other members of thehealthcare team, to help them cope with the social, emotional, and economicissues related to their illness and dialysis treatment, so they might enjoy asoptimal quality of life and independence as possible.

ESSENTIAL DUTIES:

1. Pre-dialysiseducation and assessment

2. OngoingBiopsychosocial Assessment (including quality of life measurement)

3. Casework(counseling and conferences with patients, families, and support networks;crisis intervention goal-directed counseling; discharge planning)

4. Group work(education, emotional support, self-help)

5. Mediation

6. Facilitation of community agency referrals

8. Multidisciplinary care planning andcollaboration

9. Advocacy on patients' behalf within thesetting and with appropriate local, state and federal agencies and programs

10. Patient andfamily education

11. Serve onorganizational committees.

12. Participatein Quality Assurance Performance Improvement programs

13. Participatein Multidisciplinary team meetings and Medical Director group meeting

14. Completeorganizational education

15. Assist patientsand families with Medicare, MaineCare, and other insurance benefits to assure paymentof care

16. Locateresources for adequate nutrition, housing, and medication

20. Reviewdepartment compliance with state and federal guidelines

AVAILABILITYREQUIREMENTS:

Regular part-time hours (20 hours per week) scheduled between the hoursof 0600-1700 Monday through Saturday

EDUCATION SKILLS, EXPERIENCEREQUIREMENTS:

1. Master'sdegree in Social Work from an accredited school of Social Work, required

2. Previous experience in healthcare, social orhuman service agencies, preferred

4. Strongwritten and verbal communication skills

5. Ability to motivate others and work as a teammember

6. Ability to self- motivate and to prioritize

7. Ability to work in a fast-paced medicalsetting with a diverse population

8. Ability to integrate Social Work values,knowledge, ethics and skill in a professional

setting

INTERPERSONAL SKILLS REQUIREMENTS:
