||GENERAL DESCRIPTION/STATEMENT: The dialysis social worker works with dialysispatients and their families, in collaboration with other members of thehealthcare team, to help them cope with the social, emotional, and economicissues related to their illness and dialysis treatment, so they might enjoy asoptimal quality of life and independence as possible.ESSENTIAL DUTIES:1. Pre-dialysiseducation and assessment2. OngoingBiopsychosocial Assessment (including quality of life measurement) 3. Casework(counseling and conferences with patients, families, and support networks;crisis intervention goal-directed counseling; discharge planning) 4. Group work(education, emotional support, self-help) 5. Mediation6. Facilitation of community agency referrals 8. Multidisciplinary care planning andcollaboration9. Advocacy on patients' behalf within thesetting and with appropriate local, state and federal agencies and programs10. Patient andfamily education 11. Serve onorganizational committees.12. Participatein Quality Assurance Performance Improvement programs13. Participatein Multidisciplinary team meetings and Medical Director group meeting14. Completeorganizational education15. Assist patientsand families with Medicare, MaineCare, and other insurance benefits to assure paymentof care16. Locateresources for adequate nutrition, housing, and medication20. Reviewdepartment compliance with state and federal guidelinesAVAILABILITYREQUIREMENTS: Regular part-time hours (20 hours per week) scheduled between the hoursof 0600-1700 Monday through SaturdayEDUCATION SKILLS, EXPERIENCEREQUIREMENTS:1. Master'sdegree in Social Work from an accredited school of Social Work, required2. Previous experience in healthcare, social orhuman service agencies, preferred4.
