Job Summary:

TheIntegrated Behavioral Health Coordinator (IBHC) reports to the Program Managerof the Behavioral Health Home. The IBHC ispart of an integrated multi-disciplinary team with adults who have serious andpersistent mental illness and children with serious emotional disturbance. Aspart of the integrated multi- disciplinary team the IBHC will role will bewithin physician practices, community locations, and patient's homes. The IBHC transports clients for clinicallyappropriate reasons.

The IBHC will provide comprehensive casemanagement services with a set of priorities aimed at; improving mental health,improving medical health (including ascertaining and effectively working with aprimary care provider and engaging in preventative and health promotionservices); reducing inpatient hospitalizations and emergency department visits;ensuring effective transitions during episodes of more intensive treatment;tracking client progress towards goals; and working within the team structureto continually refine and re-prioritize activities.

Job Requirements:
  • Bachelor's Degree Human Services or related field required, MHRT/C (Mental Health Rehabilitation Technician) or certification eligible within a reasonable period of time.
  • Job related experience in mental healthcare is preferred with knowledge of psychiatric and chemical dependency terminology.
  • Cognitive requirements including the ability to analyze a situation and make a sound decision on the best way to arrive at a solution.
  • The ability to work with both clinical and non-clinical personnel in potentially high stress situations is required.

Physical Demands:

Visual acuity, colorvision, hearing acuity, and tactile sensation are necessary to respond topatient response and requests. Has the ability to read and speak English, tocommunicate and document information.Possess the manual dexterity and a degree of body strength andflexibility to respond to aggressive patient behavior. Has the ability to reach and stretchintermittently. Time may be spent sitting at a computer, collaborating withother healthcare team members. Standing and walking comprises some of theworkday. Possess the ability to sit forlong periods of time in meetings and in vehicles.
