Job Details Northern Light Home Care and Hospice has a great position in our Hancock county location for a nurse practitioner wanting to make a positive difference in someone's life! You would be responsible for performing face to face encounters with Hospice clients, assisting the Hospice Interdisciplinary Team in determining ongoing hospice eligibility. You would focus on client safety, ensuring timely and correct disposition of all patients, including appropriate coordination of care and client self-management opportunities. Utilizing the latest information technologies and telecommunication systems, HCH provides you with a variety of nursing experiences, ranging from caring for clients with medical/surgical needs as well as caring for clients with hospice and palliative care needs. In addition,you would play an integral role in collaborating with other members of the client care team. The right candidate does need to have at least three years nursing experience in either family care, palliative, hospice or acute care.
