||Job Summary:The High-Risk Breast Registered Nurse Navigator is responsible for the coordination and management of care for patients in the High-Risk Breast Program. Coordinates care for patients throughout the continuum of care from initial identification of potential risk through referral to next level of care or additional follow up. Assists in obtaining and validating personal and family medical histories and other associated hereditary risk factors. Serves as a single point of contact for breast surgeons, radiologists, patients, referring physicians and community seeking information about or referral to the program. Assists providers and patients with scheduling, referrals, patient education, follow-up, hand-off communication, and coordination of services. Works collaboratively with the Imaging Center, Surgeons and Medical Oncologists. Is active in developing strong relationships and partnerships within the organization to improve patient outcomes. The incumbent works closely with physicians to develop data processes/analysis to track quality metrics and patient outcomes. The incumbent is responsible for participation in and completion of all patient safety initiatives appropriate to the position and conducts all job responsibilities according to the Mission and Values of Northern Light Mercy Hospital.Job Requirements:Valid State of Maine RN license and BLS.
Registered Nurse (RN), graduate of an accredited school of professional nursing. BSN preferred.
Minimum 2 - 4 years nursing experience required. Nursing experience in oncology, women's health, breast health, or genetics preferred. Experience in genetics strongly preferred. Must possess strong customer service and communication skills. Must possess characteristics of self-motivation, initiative and flexibility in response to changing organizational needs. Ability to easily navigate EHR systems and various other electronic documentation platforms.
