||$Sign-On Bonus$ for experienced RN meeting eligibility criteria. Our Southern Maine location has a great position for a registered nurse wanting to make a positive difference in someone's life! Under the direction of the department leader you would be in an expanded nursing role to initiate and facilitate the flow of patient information for all hospice patients statewide. This includes obtaining intake admission information on new patients, scheduling clinical staff visits when indicated, addressing patient concerns/needs, obtaining physician admission orders and collaborating with visiting staff working in the field to ensure patient care needs are met ina timely manner. Additionally, you would be responsible for providing skilled nursing services to pediatric and adult patients in their homes through assessment and development and implementation of a plan of care that integrates the patient's needs, education pathways, clinical guidelines and diagnostic tests. Your nursing skills would be optimally utilized through collaboration with primary care providers in multiple practice sites to facilitate clinical guideline adherence and coordination of services specifically related to the patient's healthcare needs. Utilizing the latest information technologies and telecommunication systems, we provide you with a variety of nursing experiences, ranging from caring for patients with medical/surgical needs as well as caring for patients with hospice and palliative care needs. In addition you would play an integral role in collaborating with other members of the patient care team. Position location is flexible.
Bachelor's degree from an accredited school of nursing or the equivalent in professional knowledge and experience
Certification (or willing to obtain within one year of hire) in Nursing Case Management by the American Nurses Credentialing Center (ANCC) or other relevant certification (for example Certification as a CHPN by the Certified Hospice and Palliative Care Nursing Certification Board
Four years nursing experience, three years of which are in palliative care and/or hospice experience
Clinical training and work experience sufficient to support strong background in care management, continuing care and transitions of care
Current ME license to practice as a registered professional nurse
Current BLS for Healthcare Providers
Computer competency and evidence of effective communication skills, using verbal, written and telephonic means; ability to make decisions regarding priority of work, strong interpersonal skills, and ability to understand specific instructions
Valid driver's license with reliable transportation and agency required auto liability insurance
Ability to travel in assigned service area.
