$Sign-On Bonus$ for experienced RN meeting eligibilitycriteria. Are you a registered nurse wanting to make a positivedifference in someone's life? Under the direction of the Palliative Caredepartment leader, you would be in an expanded nursing role to telephonicallycoordinate and provide disease management, education, and clinical managementfor patients served by the organization. This includes responsibility for casemanagement of patients living with a serious illness.

As a member of an interdisciplinary team, your nursing skillswould be optimally utilized through collaboration with care providers inmultiple practice sites to facilitate clinical guideline adherence andcoordination of services specifically related to the patient's healthcareneeds. Utilizing the latest information technologies and telecommunicationsystems, we provide you with a variety of nursing experiences, caring forpatients with hospice and palliative care needs throughout the care continuum.In addition, you would play an integral role in collaborating with othermembers of the patient care team. Position location is flexible.

Bachelor's degree from an accredited school ofnursing or the equivalent in professional knowledge and experience preferred.
Certification (or willing to obtain within oneyear of hire) in Nursing Case Management by the American Nurses CredentialingCenter (ANCC)
Four years nursing experience, two years of whichare in home health, palliative/hospice or care management preferred.
Clinical training and work experience sufficientto support strong background in care management, continuing care andtransitions of care.
Current ME license to practice as a registeredprofessional nurse
Current BLS for Healthcare Providers
Computer competency and evidence of effectivecommunication skills, using verbal, written and telephonic means; ability tomake decisions regarding priority of work, strong interpersonal skills, andability to understand specific instructions
Valid driver's license with reliabletransportation and agency required auto liability insurance
Ability to travel in assigned service area.
