Job Details Summary of Major Functions

This position is responsible for assisting the Program Director in managing the activity program on the Dementia Unit. The Assistant will help plan activity programs to carry out the objectives of the memory care unit, as a part of total resident care, and will supervise and protect residents during activities. The Assistant will learn all phases of the dementia activity programming, and will perform all duties as required, for all residents on the unit.

Essential Functions and Work Role Responsibilities

1. Help with planning, and will prepare a monthly activities calendar.

2. Transports residents to and from activities.

3. Sets up and cleans up after activity programs.

4. Maintains a general record of residents' activities.

5. Evaluates residents' responses during activities.

6. Maintain resident activity records in the electronic charting record.

7. Decorates the unit for holidays and special events, with assistance of residents.

8. Helps plan parties and provide games.

9. Helps maintain a clean, orderly, and safe work area.

10. Helps deliver resident mail, and assists them as needed.

11. Knows fire and safety regulations

12. Attends in-service programs, including dementia training.

13. Will report to the Program Director of the Dementia Unit.

14. Will assist in the dining room at meal times, including feeding of residents.

Physical Environment

Works in a modern, well-equipped environment. Stresses of supervision and deadlines incumbent to position. Exposure to blood and body fluids and possible injury from combative residents.

Physical Criteria/Effort

Physical effort required. Many duties require ability to lift a minimum of 50 pounds, stooping, bending, pushing and pulling. Frequent periods of concentration requiring close attention to detail. Frequent opportunity for diversification of tasks with no relief.


1. Maintain a safe environment which includes knowing and understanding facility and departmental policies and procedures.

2. Report and directly address identified environmental hazards when appropriate.

3. Report and directly address violations of resident safety policy and/or protocol when involved or observed.

Occupational Risks

Failure to follow established procedures could result in undue exposure to safety hazards and/or disease/infection in resident contact.


Valid driver's license or reliable transportation. Ability to work well with others and to function effectively without supervision. Ability to supervise. Cleanliness and proper dress. Maintains current and active Certified Nursing Assistant certification.
