||POSITION SUMMARY: This position works in conjunction with the ERAS (Enhanced Recovery After Surgery) Physician Champion. Responsibilities include care coordination for ERAS patients through all stages of the peri-operative continuum ensuring the pathway is followed by all disciplines. Ensures ongoing ERAS education to the patient/family, care team and community resources, identifying and resolving clinical issues which affect patient care, clinical efficiency, effectiveness and making programmatic improvements related to outcomes through collaboration, project management and performance improvement activities. Exemplifies the EMHS/EMMC mission, vision and values and acts in accordance with EMHS/EMMC policies and procedures.MINIMUM EDUCATION REQUIRED: Bachelors Nursing (BSN) MINIMUM EDUCATION PREFERRED: Masters Nursing (MSN) MINIMUM EXPERIENCE REQUIRED: Five years professional nursing experience with progressive clinical experience including care of surgical Patients and care management. Current valid Maine Registered Nurse license or eligible for licensure. BLS Certificate (Basic Life Support)OTHER SKILLS REQUIRED: Ability to facilitate and lead a team approach to population-based care as needed. Excellent written and verbal communicatin skills. Strong interpersonal, delegation and problem solving skills. Strong analytical and data management skills. Project managment experience. Detail oriented with strong organizational and prioritization skills. Ability to work independently under general guidance. Proficiency with electronic medical records (EMR). Basic proficiendy with Microsoft Outlook, Word and Excel.PHYSICAL REQUIREMENTS: Visual acuity, hearing acuity, and tactile sensation are necessary. The ability to read English, manual dexterity, visual acuity, and flexibility. Standing or walking comprises 90% of the workday. 10% of the time may be done sitting at the computer, use of phone, and collaborating with other health team members. The job involves a moderate amount of stooping, pushing, lifting, bending, and reaching and a large amount of standing. WORKING ENVIRONMENT: Hospital/office setting
