This includes responsibility forproviding skilled nursing services to clients in their homes through assessment and development and implementation of a plan of care that integrates the client's needs, education pathways, clinical guidelines and diagnostic tests.Your nursing skills would be optimally utilized through collaboration with primary care providers in multiple practice sites to facilitate clinical guideline adherence and coordination of services specifically related tothe client's healthcare needs. Utilizing the latest information technologies and telecommunication systems, VNA provides you with a variety of nursing experiences, ranging from caring for clients with medical/surgical needs as well as caring for clients with hospice and palliative care needs. In addition you would play an integral role in collaborating with other members of the client care team.Bachelor'sdegree from an accredited school of nursing or the equivalent in professional knowledge and experience
Certification (or willing to obtain within one year of hire) in Nursing Case Management by the American Nurses Credentialing Center (ANCC)
Four years nursing experience, two years of which are in home health care or care management
Clinical training and work experience sufficient to support strong background in care management, continuing care and transitions of care.
Current ME license to practice as a registered professional nurse
Current BLS for Healthcare Providers
Computer competency and evidence of effective communication skills, using verbal, written and telephonic means; ability to make decisions regarding priority of work, strong interpersonal skills, and ability to understand specific instructions
Valid driver's license with reliable transportation and VNA required auto liability insurance.
