This is an exciting and challenging opportunity to join a growing team of integrated behavioral health providers, bringing tools and resources into the primary care practice to support the health of the community!

The Integrated LCSW position provides a great opportunity to use evidence-based knowledge and acquired skills as a key member of a dynamic, collaborative team in an integrated primary care setting. This person is a clinical leader, consultant and resource at the medical practice in program development, in guiding group decision-making, in implementing plans, and improving quality of care. He/she is an enthusiastic learner who actively seeks research and knowledge to ensure current, evidence based practice while adhering to key components of our integrated behavioral health model.

The Integrated LCSW will be skilled in establishing rapport and maintaining effective working relationships with diverse individuals, including patients, family members, and other providers. The integrated LCSW serves as an effective member of an inter-professional team, helping other providers on the team to quickly conceptualize a patient's strengths, healthcare problems, and an appropriate plan of care. He/she adapts interventions to the work flow and pace that typically characterizes the provision of primary care, including rapid assessment, brief treatment, and a high daily volume of patient contacts. He/she advocates for, teaches, and supports whole health self-management and recovery approaches.

He/she will demonstrate a fundamental belief in the value and effectiveness of brief interventions to improve health through practice patterns and communications with patients, family members, and other members of the health care team. He/she collaborates with the healthcare organization, behavioral health leadership, and other local healthcare agencies to continuously assess and improve service system design. The integrated LCSW has ready access to clinical and administrative supervision, and the generous benefit package includes ongoing learning opportunities.
