Northern Light HomeCare & Hospice is seeking per diem Nurse Practitioner in the Hancock county area of Maine. The Nurse Practitioner is responsible for performing face to face encounters with Hospice clients and assisting the Hospice Interdisciplinary Team in determining ongoing hospice eligibility. Focus is on client safety, ensuring timely and correct disposition of all patients, including appropriate coordination of care and client self-management opportunities. Utilizing the latest information technologies and telecommunication systems, Home Care and Hospice provides a variety of nursing experiences, ranging from caring for clients with medical/surgical needs as well as caring for clients with hospice and palliative care needs. This position would play an intricate role in the patients' care and would be well-involved in collaborating with other members of the client's care team.

Qualification Requirements:

·Bachelor's degree from an accredited school of nursing

·Active State of Maine APRN license

·Certification by the national certifying bodyappropriate to his/her clinical specialty

·Three years acute care nursing experience in hospice/palliative care

·Current BLS for Healthcare Providers

·Computer competency and evidence of effective communication skills, using verbal, written and telephonic means; ability to make decisions regarding priority of work, strong interpersonal skills, and ability to understand specific instructions

·Valid driver's license with reliabletransportation and HCH required auto liability insurance

·Ability to travel in assigned service area.
