and Western Washington counties.POSITION SUMMARY: Directs and provides professional nursing care while meeting patients' psychosocial, physical and general aspect of care needs. Coordinates care planning with other disciplines, families and/or caregivers. Responsible for initial assessment, reassessment and documentation of assigned patients. Determines priorities of care based on physical and psychosocial needs of patients. Communicates with practitioners regarding changes in patient's status, symptoms and results of diagnostic studies. Responds quickly and appropriately to changes in patient condition or response to treatment. Assumes responsibility for supervising/assessing other nursing personnel. EDUCATION REQUIREMENTS: Graduate of an accredited school of nursing, BSN preferredEXPERIENCE REQUIREMENTS: Two years of acute care experience preferred. Preference given to nurses with previous practice community health outpatient nursing experience. Nursing preferred
